Liability Wavier  

In signing, I agree and attest I am attending a class with the above mentioned partners, open to anyone who wishes to participate of my own free will. The purpose of this class is not to diagnose or treat. While the class is lead by trained individuals, participation in the class carries with it a risk of injury that cannot be eliminated regardless of the care taken to avoid such injuries and the undersigned individual shall assume the risk by participating in the class. By reserving a spot and in consideration for the aforementioned class, I do waive and forever release any and all right and claims for damages and liabilities of any kind arising out of my participation in this class, including but not limited to Alpha Osteopractic, LLC, its owners, employees, contractors, independent instructors, and any other related individuals or entities. I understand that this class is not intended to diagnose or treat any injuries. I also understand that my participation in this workshop is not a guarantee of injury prevention or performance. I also agree to being contacted by Alpha Osteopractic, LLC in the future. We will not share your personal information hereunto per Alpha Osteopractic, LLC's Privacy Policy. I have carefully read this waiver of liability and understand that I am giving up legal rights, including my right to sue. I acknowledge that I am reserving a spot freely and voluntarily and intend by my reservation to be a complete and unconditional release of all liability to the extent allowed by law.

Media Release Consent

By attending this event, I grant permission to Alpha Osteopractic, LLC to take photographs and/or video recordings that may include my image, likeness, or participation. I understand that these materials may be used for promotional, educational, or marketing purposes including, but not limited to, social media, websites, printed materials, and other digital platforms. I waive any right to inspect or approve the final use of such media. I acknowledge that no compensation will be provided and that all media content will be the exclusive property of Alpha Osteopractic, LLC.
